All payments are due at the time of services. Payments can be made by check, cash, debit/credit card or certain Health Insurance Spending Account cards.
Payment is made directly to me at the time of your session, but your insurance company may reimburse you. It is your responsibility to check with your insurance company before our first session to have a clear understanding of how they will reimburse for an out-of-network provider. Click on the link below for the Health Insurance Claim Form in order to submit to be reimbursed or for your payment to count towards your annual deductible. You will need certain codes from me to complete in order to be accepted by your insurance and I'm happy to fill those portions out and return to you.
As per the legal contract that you will sign before our first session, I require at least 24-hours notice to be given to cancel or reschedule an appointment. Failure to give 24 hours or a missed appointment will be charged the full session fees stated above.
